| On your first visit you will be asked to complete a registration form, and review and sign a Govrment also be asked to fill out a questionnaire describing your health history and identifying the reason for your visit. These forms will take several minutes to complete, so we ask that you arrive at least 30 minutes ahead of your appointment. |
| We are committed to excellence in patient care. We believe our care is dependent on your understanding of the diagnosis and treatment options for you and/or your family member. We make a special effort to explain your situation in a way that you and your family can understand. If the information is not clear, please ask. We encourage your questions and strive to meet your needs. |
| After seeing your doctor, you may require lab tests or other diagnostic tests. Due to insurance restrictions, your primary care doctor and insurance carrier often must approve these before they can be ordered. |
| We will try to contact you with the results of your tests once they have been completed and returned to us. If you have not heard from us within 10 days, please call our office. |

| Please call your pharmacy before you run out of your medication. After of Refill requests for patients who have not been seen for more than one year will not be filled. |
