What to Expect at Your Visit

On your first visit you will be asked to complete a registration form, and review and sign a Govrment also be asked to fill out a questionnaire describing your health history and identifying the reason for your visit. These forms will take several minutes to complete, so we ask that you arrive at least 30 minutes ahead of your appointment.
We are committed to excellence in patient care. We believe our care is dependent on your understanding of the diagnosis and treatment options for you and/or your family member. We make a special effort to explain your situation in a way that you and your family can understand. If the information is not clear, please ask. We encourage your questions and strive to meet your needs.
After seeing your doctor, you may require lab tests or other diagnostic tests. Due to insurance restrictions, your primary care doctor and insurance carrier often must approve these before they can be ordered.
We will try to contact you with the results of your tests once they have been completed and returned to us. If you have not heard from us within 10 days, please call our office.

Prescriptions

Please call your pharmacy before you run out of your medication. After of Refill requests for patients who have not been seen for more than one year will not be filled.
 

Billing Questions

For billing questions call 98661-57722. As a country to you ,we will major primary and secondary insurances. The credit policy you receive as a new patient gives you detailed information about our billing policies.

Appointments

Office visits are by appointment and often require a referral by your primary care physician.
Cancellations: If you are unable to keep your appointment, please call the clinic at least 24 hours in advance of cancellation. We will call you two days before your appointment to remind you of your visit and leave any necessary instructions. It is important that you provide us with a daytime telephone number so that we can reach you. Since our physicians are surgeons, occasionally an appointment may need to be changed due to an emergency. We will try to give you as much notice as possible should this occur.

Clinic Office Hours

10:00am to 6:30pm Monday through Saturday. The clinic is closed on major holidays and Sundays.

Calling the Office

Our Staff can be reached Monday to Saturday by calling the main office number of the clinic where you receive your care. When you call, you will speak to a receptionist who will help to answer your questions. If your question is of a clinical nature, the receptionist will take a message and have the nurse or doctor return your call. Calls are generally returned within two to four hours.

Emergency Calls

If you have a life-threatening emergency, call 108 or go to the nearest emergency hospital. If your call is urgent, please let the receptionist know this when your call is answered. If the clinic is closed and you call after-hours, the answering service will take your message and put you in touch with the provider on call for that day.
